前言

  Acupuncture is an ancient Chinese therapy with an extensive history, playing animportant role in traditional Chinese medicine. Now it has been widely spread tomany parts of the world, although there are still doubts and misgivings in people whoare unfamiliar with the practice. Based on my previous experience as a practicingclinician, I truly believe that acupuncture is effective for certain diseases andconditions, especially those related to neurological dysfunction/disorders. Becauseof its convenience, low cost, and few side-effects, acupuncture is worthy to try as amedical modality, even in the modern medical system. This is especially true forneurology, internal medicine and other medical specialties without surgical operation.Indeed, its value has been increasingly recognized in not only the eastern countries,but also in the western world. For example, after the 1997 NIH hearing in termsof its efficacy, acupuncture has become one of the most popular therapies incomplementary and alternative medicine in the USA.  The mechanisms of acupuncture, however, are not yet well understood, whichis a major obstacle in the way of the promotion and improvement of acupuncturetherapy. In the past 5 decades, many Chinese scientists, especially those fromShanghai and Beijing, have made significant efforts in clinical and basic researchon acupuncture and have published substantial data which sheds some light on themystery of acupuncture. Indeed, two of the editors of this book, Profs. Cao andCheng, have been engaged in acupuncture research since the late 1950s. In fact,Prof. Cao had been one of my mentors during the time when I conductedacupuncture research in my graduate study at Shanghai Medical University (nowShanghai Medical College of Fudan University) in the early 1980s.

  In the history of Chinese medicine, acupuncture has been used for a long time torelieve cardiovascular symptoms, such as palpitations, vertigo, choking sensationin chest, and precordial pain observed with cardiovascular diseases. In the past30 years, several studies performed in our institution have provided strong evidenceon the efficacy of acupuncture on cardiovascular diseases, such as cardiac arrhsNunia,hypertension, and hypotension. The underlying mechanisms have also been well-addressed by experimental research. The massive information obtained demonstratethat the efficacy of acupuncture on cardiovascular diseases is dependent on thediseases treated, acupoints selected, needling manipulation, stimulus intensity,applying windows, and the time of treatment. The mechanistic research hasdeveloped the idea that acupuncture signals initiated at the acupoints aretransferred to the brain through the nervous pathway, which modulate multipleneurotransmitter systems, thus, affecting the nerve output and regulating thecardiovascular system. In addition, acupuncture has been observed to affect theendocrine secretion as well as humoral and dielectric regulation. Moreover, thesemechanisms are observed to be involved in the alterations of gene expression andintracellular modulation of signal transduction. Thus, acupuncture is observed toinduce an integrated and complex effect on the cardiovascular system, which isdependent on multiple factors. The details of acupuncture effects on cardiovascular disorders are presented inChapters 10 and 11 of this book.
